AŞİL TENDON YIRTIKLARI TEDAVİSİNDE AÇIK ve PERKÜTAN TEKNİKLERİN KARŞILAŞTIRILMASI

Abstract

Akut aşil tendon yırtıkları
ayak bileği çevresindeki en sık
karşılaşılan tendon yaralanmasıdır. Aşil tendon
rüptürü
olan hastalarda tedavi konusunda
halen litaretürde  ortak 
bir  görüş   yoktur.   Bu çalış mada aşil tendon yırtığı tedavisinde iki farklı tekniğin fonksiyonel  sonuçları 
karşılaştırıldı. Çalış maya
2010 ile 2017 tarihleri arasında açık
cerrahi ve perkütan cerrahi teknikle tedavisi yapılan
31 hasta dahil edildi. Hastaların son
kontrollerinde ayak bileği eklem hareket açıklığı, dorsifleksiyon ve plantar fleksiyon güçleri, işe ve spora geri dönme süreleri
değerlendirildi. Hastaların fonksiyonel değerlendirilmesi
Amerikan Ortopedik Ayak ve Ayak Bileği Derneği’nin (AOFAS), ayak bileği eklemini değerlendiren skorla ma
siste mi kullanıldı. Çalış maya alınan hastaların 29’u erkek 2’si bayan id
i.
Üç hasta trafik kazası, 26’sı spor
yaparken, 2’si ise basit düşme sonrası rüptür
geliştiğini b ild irdi.
Hastaların 18’ine açık cerrahi teknik
(Grup 1), 13’üne perkütan cerrahi teknik
(Grup 2) uygulandı. Grup 1’de bir hastada derin ven trombozu, üç hastada yüzeyel doku enfeksiyonu gelişti. Grup 2’de ise iki hastada sural sinir duyu alanında hipoestezi izlendi. Her iki grubun AOFAS skorlama sistemine göre
aralarında
istatistiksel fark bulunmadı. Aşil tendon yırtığı
nedeniyle opere olan hastalarda açık
cerrahi teknik ve perkütan cerrahi teknik
arasında fonksiyonel
sonuç olarak
anlamlı bir farkın olmadığı kanaatine varıld
ı.

Comparison of open and percutaneous techniques in the treatment of achilles tendon tears

Abstract

Acute
tear of the Achilles tendon
is the most common tendon injury
around the ankle. There is no consensus on what method is more effective
in the
treatment of Achilles tendon rupture.
The
aim of th is study
is to compare the functional
results of two different
techniques in the treatment of achilles tendon rupture. Between
2010 and 2017, 31 patients who had undergone
open surgery and percutaneous surgery because of achilles rupture were
included in the study. In the last exa
minations of the patients, the range of motion of the ankle,
the duration of return to work and sports
were evaluated. Functional
evaluation of patients The American Orthopedic Foot and Ankle
Association (AOFAS) scoring system
was used to evaluate the ankle joint. Of the 31 patients
included in the study, 29
were male and 2 were female. 26 of them were sports injuries, 3 of them after traffic accident,
and 2 of them after a simple fall. 18
patients underwent open surgical technique (Group 1) and 13 patients
underwent
percutaneous surgical technique (Group 2). There was no statistical difference between the
two groups according to AOFAS scoring
system. It was concluded that
there was no significant difference between the
open
surgical technique and the percutaneous surgical technique in
the Achilles tendon tear.
